Despite Albertans believing in the benefits of advance care planning, and taking steps to complete it, their wishes may not influence decision-making in healthcare settings.  Our panelists will explore how advance care planning commonly breaks down in the hospital setting, and lead a discussion on potential solutions.  Do recently-developed joint medical-legal approaches and resources have merit?  Provide your insights and help us strengthen legal and medical practices!  This session is moderated by Maureen Douglas and Patricia Biondo (ACP Project: Alberta Medical-legal Collaboration, Covenant Health Palliative Institute).
