It's About Living: Promoting Advance Care Planning
Presented by the CBA Alberta Elder Law North Section
On Demand. Originally presented on February 5, 2024.
This presentation will be structured around three main questions: 1. What is the current understanding of optimal advance care planning ("ACP"), and how can lawyers facilitate their clients in completing it? 2. What are health care providers'/health systems' roles in ACP? What might lawyers better understand about patient care, treatment, practices (e.g. Serious Illness Conversations Guide) and policies/procedures (e.g. Goals of Care Designations, electronic records) that would impact their practice? 3. Given each profession has a role in ACP, how do we work together to help our clients/patients reach their goals?
SPEAKERS
Dr. Charlie Chen (Medical Lead for Advance Care Planning and Goals of Care for the Calgary Zone)
Maureen Douglas (Lawyer and Consultant for the Advance CAre Planning Alberta Prokect)
